Strong Bones, Stronger Living

Why Bone Health Matters
As with other age-related conditions, bone loss occurs gradually. Many people aren’t even aware they’ve developed osteoporosis until a fracture occurs–and often, those fractures are small spinal fissures that cause sudden back pain. As osteoporosis progresses, it can also cause other physical complications, such as a stooped posture that can impact balance and stability.
For facility operators and care teams, the risks for their patients are clear:
- Increased falls
- Higher hospital readmissions
- Decline in functional independence
However, therapy plays a key role in addressing these risks early and consistently.
How Sobe Rehab Supports Stronger Bones and Better Function
Our multidisciplinary model—which includes physical, occupational, and speech therapy—helps residents maintain strength, improve movement, and stay engaged in daily life. And because we deliver therapy directly in assisted living facilities, we can encourage greater resident engagement and fewer transportation barriers, enabling true continuity of care.

Physical Therapy: Strength, Balance, and Fall Prevention
Targeted exercise is one of the best ways to slow bone loss and improve overall function, making physical therapy an essential part of preventing and managing osteoporosis. When working with residents, our PTs focus on safe movement and strength building.
Key interventions include:
Weight-bearing exercises
such as sit-to-stands, to stimulate bone growth
Resistance training
using light weight or resistance bands to help build both muscle and slow bone loss
Balance training
to reduce fall risk by improving stability and coordination.
Postural correction
to address postural issues that put strain on the spine and lower the risk of fractures.
Functional movement training
that allows residents to practice real-life tasks, such as transfers and walking
Occupational Therapy: Safe, Independent Daily Living
Occupational therapy builds on the foundation established by physical therapy, helping residents continue daily activities safely, even with reduced bone density. It promotes dignity among residents while reducing caregiver burden.
Focus areas include:
ADL (activities of daily living) training
to promote dressing, bathing, and grooming techniques that reduce strain and fall risk
Energy conservation strategies
to help residents complete tasks without fatigue, lowering injury risk
Recommendations for safer room setups
such as safer seating or furniture arrangements
Training in adaptive equipment
to improve safety and independence during routine tasks
Speech Therapy: Cognitive and Swallowing Support
Speech therapy plays a supporting role in managing osteoporosis, especially for residents with cognitive decline or swallowing issues. Maintaining proper nutrition is essential for preserving bone density and overall strength.
Key benefits include:
- Cognitive training to improve attention and safety awareness, which can reduce fall risk
- Swallowing therapy to ensure safe intake of calcium- and vitamin D-rich foods, which are critical for bone health
Supporting Bone Health in Hospice Care
Bone health can also be an important consideration in hospice settings, where our therapists focus on comfort, safety, and quality of life.
Gentle mobility exercises help prevent stiffness and reduce pain, while proper positioning protects fragile bones and minimizes the risk of pressure injuries. Even small movements can support circulation and comfort.
Our therapists also guide caregivers on safe transfers and daily care techniques to lower fracture risk.
This approach helps patients remain as comfortable and functional as possible.
Building a Stronger Foundation for Independence
Improving bone health isn’t just about reducing fractures. It also promotes safer mobility and greater confidence in movement, which in turn helps residents participate more fully in daily life.
Early intervention and consistent therapy can slow decline and preserve independence. Through targeted, evidence-based care, the Sobe Rehab therapists can help residents move safely, maintain function, and stay engaged in the activities that matter most, right where they live.

Simple Ways to Stay Steady and Active
Staying active is one of the most effective ways to reduce fall risk and maintain independence in assisted living. Even better: small, consistent habits can improve balance, strength, and confidence with movement.
One way the physical, occupational, and speech therapists at Sobe Rehab help patients is by providing simple, personalized strategies to become more active. Today, we’re sharing some of our favorite tips to pass along to residents.
First Things First: Staying Safe as You Stay Active
- Wear supportive footwear. Shoes with non-slip soles reduce the chance of slipping and falling.
- Remove clutter and ensure adequate lighting in living spaces to minimize trip hazards.
- Ensure you’re using assistive devices, such as walkers or canes, correctly. Our physical therapists can provide guidance as needed.
Next, Work on Building Strength and Mobility
- Sit-to-stand exercises, which involve repeatedly standing up from a chair, help build leg strength and support safer transfers.
- Spend a few minutes each day working on balance. Try standing with feet together or shifting weight side to side while holding a stable surface.
- Practice good postural awareness. Maintaining proper posture reduces strain and helps maintain your center of gravity.
Add More Movement Throughout Your Day
- Fold laundry or organize personal items while standing to build endurance.
- Short, supervised walks improve circulation, endurance, and coordination. Try walking to meals and activities or to visit a neighbor.
- Look at daily activities, such as brushing teeth, getting dressed, and light cleaning, as opportunities for movement.
- Join group activities that involve movement to improve consistency and motivation.
Confidence Starts with Consistent Movement
Building strength and balance supports safer, more confident movement. Sobe Rehab provides personalized balance and fall prevention programs on-site. Contact us today to learn more.
