Meet the Team Behind Sobe Rehab’s Continued Growth

A Multidisciplinary Team Built for ALF Excellence
Whether supporting mobility and balance, daily living skills, communication, or cognitive engagement, our clinicians work closely with ALF staff and caregivers to help residents remain active, confident, and engaged within their community.
Sobe Rehab’s clinical team includes Physical Therapists, Occupational Therapists, Speech-Language Pathologists, Therapy Assistants, and support clinicians who specialize in senior-focused care. Our therapists are deeply experienced in working within Assisted Living environments, where collaboration, communication, and consistency are essential.
The Leadership and Support Teams That Keep Everything Moving
Behind every successful therapy visit is a strong operational foundation. Our leadership, onboarding, compliance, scheduling, EMR, and administrative teams ensure that services run smoothly for residents, facility partners, and referral sources alike.
These teams are responsible for:
- Coordinating care across multiple facilities and regions
- Supporting therapists with documentation, training, and technology
- Streamlining onboarding and authorization processes
- Strengthening partnerships with ALFs, hospice agencies, and referral partners
Their work allows our clinicians to stay focused on what matters most: delivering exceptional care.
Recent Initiatives Driving the Next Chapter
Over the past year, Sobe Rehab has made several strategic investments to support long-term growth and sustainability. These initiatives include expanding hospice staffing partnerships, developing in-home therapy services for Personal Injury Protection cases, strengthening internal training and infrastructure, and preparing for broader managed care participation.
While these efforts required time and focus, they were designed to build a stronger, more resilient organization that can continue to serve partners and patients at a high level well into the future.

One Team, One Mission
No matter the role, department, or setting, the Sobe Rehab team shares a common purpose: supporting independence, stability, and quality of life for the seniors and individuals we serve. This collective commitment is what has allowed Sobe Rehab to grow while staying true to its ALF-centered foundation.
As we move forward, we are grateful for our team members, our facility partners, and the trust placed in us every day. Together, we look ahead to another year of collaboration, innovation, and continued impact.

Mission Statement:
Our mission is to deliver a stronger future for seniors in assisted living facilities. The Sobe Rehab 10-step proactive, comprehensive, PT/OT/ST therapy model restores senior independence and quality of life. In ALFs, this translates into improved length of stay and retention, on average 4 years for our patients. This enhances their freedom and happiness while increasing the ALF census exponentially. As the saying goes, “It’s not the days in your life, but the life in your days.”
